CASE scenario...

Hi I m G sukanya, 3rd bds student.this is an online elog book to discuss our patient health data after taking his consent .This reflects my patient centered online learning portfolio. 


CASE HISTORY:A 38 years old known case Resident of Ratnapuram presented with the 

CHIEF COMPLAINTS:Shortness of breathe and edema  of  feet from 2 months

HISTORY OF PRESENT ILLNESS:

Patient was apparently asymptomatic 1 week back and then had SOB and pedal edema and brought to the hospital for dialysis,only one episode of dialysis is done.

PAST ILLNESS :

 It was apparently normal 2 years back then had an accident and then taken into the hospital and found to be suffering with chronic kidney disease and it is relieved by medicine,clinidipine and hypertension is present. 

PERSONAL HISTORY:mixed diet,loss of apetite and inconvenience in sleeping due to gastric problem,constipation and proper micturition regularly. 

FAMILY HISTORY:

Diabetes:No

Hypertension:yes

DRUG HISTORY:

GENERAL EXAMINATION: 

Built:thin

Nourishment:poor

Conscious:present in Conscious 

Pallor:pale pink

Icterus:not present 

Cyanosis:No

Clubbing:no

Lymphadenopathy:No lympadenopathy

Edema:pitting edema extended upto the knee

VITALS:

Heart rate:72bpm

Respiratory rate:18cpm

Blood pressure:200/120 mmHg

Spo2:97%


               :PROVISIONAL DIAGNOSIS:

                -------RENAL FAILURE-----

Q/A

1)Am I a candidate for a kidney transplant? If so, what do I need to do?

2) Is dialysis is only the way for kidney failure ?
